Community Case Manager
Description

Provide comprehensive employment-based case management services to low-income individuals and families. Works with participants to develop appropriate case plans and goals. Provides ongoing assessment and support, assisting participants in the utilization of internal and community resources to meet the individual/family needs. Process emergency assistance services to individuals and families.

Education/Qualifications/Skills

Minimum bachelor’s degree in social work, Counseling, Psychology, Sociology, or another applicable field. In addition, a minimum of two years documented experience providing case management, outreach services, family services and/or housing placement services, preferred. *Associate degree may be considered in conjunction with documented experience. The ability to successfully organize, communicate, & manage well under pressure. Excellent computer and clerical skills. Use of Microsoft Office Software/Products. Professional use of telephone skills, working knowledge of various types of office equipment. Ability to work independently and as a team member & interact with people from all socioeconomic backgrounds. The desire to work toward the success of low-income families.

Job Requirements/Responsibilities

Provide comprehensive case management services to low-income individuals and families. Gather and evaluate participant information regarding employment history, housing situation, physical functioning, financial status, capacity for independent living. Work toward the goal of assisting individuals and families to obtain and maintain self-sufficiency. Seek and maintain strong relationships with local employers to access those seeking work and refer participants to current job openings. Manage assigned caseload. Evaluate participant concerns, observe behavior, develop service plans, and establish and maintain case files – both hard copy and electronically; enter & update required data and case notes in North Central's client-tracking database (LITT) correctly and timely. Maintain confidentiality of information. Prepare narrative and statistical reports and documents and correspondence regarding participant status. Assist Emergency Assistance clients & process client assistance payment paperwork in a timely manner - paperwork must be completed in its entirety ensuring all required documentation is included. Coordinate seasonal programs in county such as Garden Program, Forgotten Angel, VITA/EITC Program, and other seasonal and/or county programs.
Valid Driver’s license. Be able to provide own transportation; carry adequate insurance coverage; Criminal Background Check; Protective Services Record Check. Ability to drive long distances. Must be willing to travel & attend required meetings.

Physical Demands

While performing the duties of this job, the employees occasionally are required to stand, walk, sit use hands, drive an automobile, talk, and hear. The employee must occasionally lift and or move up to twenty-five pounds.
