The Community Case Manager will work with participants to develop appropriate case plans and goals and provide ongoing assessment and support, assisting participants to utilize internal and community resources to meet individual needs.

Qualifications

Bachelor’s Degree in social work, Counseling, Psychology, Sociology, or another applicable field. Two years of experience providing case management or outreach services preferred. Excellent computer knowledge, strong communication skills, and the desire to work toward the success of low-income families. Valid Driver’s License, proof of insurance, dependable personal vehicle, and background check with no criminal history. Ability to work independently.

Job Responsibilities

Provide comprehensive case management services to low-income individuals and families. Gather and evaluate participant information regarding employment history, housing situation, physical functioning, financial status, capacity for independent living toward the goal of assisting individuals and families to obtain and maintain self-sufficiency. Seek and maintain strong relationships with local employers to access those seeking work and refer participants to current job openings. Evaluate participant concerns, observe behavior, develop service plans, and establish and maintain case files in North Central’s Client-Tracking Database (FacsPro). Maintain confidentiality of information. Manage assigned case load, prepare narrative and statistical reports, and documents and correspondence regarding participant status. SSVF program (veterans’ program) responsibilities such as providing eligible families with outreach, case management, and assistance obtaining VA and other benefits.

Physical Demands

While performing the duties of this job, the employees occasionally are required to stand, walk, sit use hands, drive an automobile, talk, and hear. The employee must occasionally lift and or move up to twenty-five pounds.
