The Shelter Housing Stabilization Worker (SHSW) will facilitate Scott Place Shelter case management by providing a seamless transition of individuals experiencing homelessness into affordable, quality standard housing. The position will provide a housing-focused plan and implement stabilization services to assure a rapid transition to permanent housing. The ideal Stabilization Worker is a self-driven, empathetic professional capable of building rapport and trust with high-needs, vulnerable individuals to facilitate long term independence and stable housing. Individuals should be assertive, resilient, and hard-working, with a working knowledge of community support and resources.
Associates or bachelor’s degree from an accredited college or university in human services field preferred and/or at least 1-year documented experience in case management. Housing &/or employment-based case management working with homeless individuals preferred. Mental health &/or substance abuse related experience is a plus. Excellent Computer knowledge, preferable Microsoft Office, email familiarity. Strong written, verbal, and non-verbal communication skills, strong interpersonal and analytic skills, ability to demonstrate empathy, problem-solve interpersonal issues and client barriers. Knowledge of mental health standards and service provision is a plus. Ability to work independently and make sound decisions. Desire to work with homeless and/or low-income individuals. Previous experience with motivational interviewing is a plus.
While performing the duties of this job the employee occasionally is required to stand, walk, sit, use hands, drive an automobile, talk, and hear. The employee must occasionally lift and/or move up to 25 pounds. You can walk up and down at least 35 steps 4 or more times daily.
Social Work license preferred. Valid Driver's License. Be able to provide your own transportation. Must be able to obtain CPR/FA cards, CPI (Crisis Prevention Intervention) and any other certifications that may be required for the position. *Some certifications are provided/paid for by employer. Complete Criminal Background Check; Protective Services Record Check; TB Risk Assessment or TB screening. Must be able and willing to workday, evening, and occasional weekends as required by program needs including holidays. Complete necessary forms and paperwork on new and existing program participants. Utilize required case management tools and assessments to facilitate permanent housing. Maintain thorough and organized records of services provided in Client-Tracking Database (DBA FacsPro and HMIS) and prepare required reports regarding participant status toward self-sufficiency. Enter client data daily/weekly in DBA FacsPro/HMIS and facilitate housing stabilization plans. Meet with existing clients weekly to maintain and update case plans, evaluate participant concerns, observe behavior, and assist client implementation in assessing short and long-term goals. Link and refer participants to other mainstream community resources. Assist with removing barriers to finding long term, permanent, self-sufficient housing. Facilitate appointments (employment, medical, psychological, mainstream benefits, housing location, etc.) through transportation, client scheduling, etc. Actively attend and participate in staff meetings as required. assist client implementation in assessing short and long-term goals. Link and refer participants to other mainstream community resources. Assist with removing barriers to finding long term, permanent, self-sufficient housing. Facilitate appointments (employment, medical, psychological, mainstream benefits, housing location, etc.) through transportation, client scheduling, etc. Actively attend and participate in staff meetings as required.
