Community Case Manager
Job Summary

Provide administrative assistance to CSBG program; essentially purchasing, procurement, requisitions, inventory, and payment of services for the CSBG program. Provide client assistance through Emergency Assistance Program as well as Case Management services. Assume other county program responsibilities as assigned by the Community Services Program Manager/Community Services Director. Adhere to program reporting procedures and maintain certifications as required.

Education/Skills/Qualifications

Minimum Associates Degree in Social Work, Counseling, Psychology, Sociology, Business/Office Management or another applicable field. In addition, two years documented experience providing case management, outreach services, family services &/or housing placement services.
*Bachelor’s Degree preferred.16

Job Responsibilities

Be able to provide own transportation; carry adequate insurance coverage; Criminal Background Check; Protective Services Record Check. Ability to drive long distances. Must be willing to travel and attend required meetings. The desire to work toward the success of low-income families. The ability to successfully organize, communicate, and manage well under pressure. Be a self-starter and aggressive team member. Excellent computer and clerical skills. Professional use of telephone skills, excellent computer skills, working knowledge of all types of office equipment, and the ability to work harmoniously with other employees. Greet clients in person and by phone, take messages, understand, and explain program eligibility and guidelines. Maintenance of the county CSBG budget including monthly reconciliation of expenses and income in each budgetary line item. All client information to be entered into the client tracking program, DBA FACS PRO system, as clients are seen; update DBA FACS PRO as needed to assure timely, accurate reporting. Assist Emergency Assistance clients & process client assistance payment paperwork in a timely manner - paperwork must be completed in its entirety ensuring all required documentation is included. Assist clients in completing other various program applications as needed/requested. Maintain program finance records such as: requisitions, bills, blanket purchases, reconcile billing statements, prepare vouchers, as per agency purchasing procedures. Assist with compiling individual county/office mailings for any/all programs as necessary. Prepare
and submit all necessary reports and other such documents as required by contracts and other such directives. This must be performed in a complete and timely fashion. Maintain adequate supply of forms required for daily operations of programs.
Assist Community Services Program Manager in updating County Services Directory as needed/requested. General office organizational duties will include providing for office supplies and other sundry items including custodial supplies, equipment supplies and maintenance, etc. Attend agency or state trainings as required.
Assist Community Services Program Manager with special projects, assignments, tasks, etc. as needed/request when available. Work with the Community Services Program Manager with planning, publicizing, and implementing county seasonal programs such as: Forgotten Angel, Low Income Energy Assistance (LIEAP) Program, Garden Program, Summer Food Service Program, Volunteer Income Tax Assistance (VITA), etc. Submit a written report to the Community Services Program Manager of any program &/or property losses immediately but no later than 24 hours from the time of the incident. Adhere to all agency policy and procedures. *Other related duties as assigned. Supportive Services for Veterans and Families-Provide comprehensive case management services to low-income veterans and families. Gather and evaluate participants information regarding employment history, housing situation, physical functioning, financial status, and other program specific criteria to determine eligibility for services. Link and refer participants to other mainstream community resources. Assist with removing barriers to finding and maintaining housing and employment. Provide outreach and explore partnership opportunities. Maintain thorough and organized records of services provided in North Central’s Client-Tracking Database (DBA FacsPro and HMIS) System and prepare required reports regarding participant status toward self-sufficiency.

Physical Demands

While performing the duties of this job, the employees occasionally are required to stand, walk, sit use hands, drive an automobile, talk, and hear. The employee must occasionally lift and or move up to twenty-five pounds.
