NON-FEDERAL SHARE (IN-KIND)

Instructions for Board of Education Transportation

Form FCP#010F

 

Front line staff will use this form to document transportation provided for ACF enrollees by the Board of Education.

 

To be completed for each location on October 31 by front line staff and submitted to immediate Supervisor in November each year:

 

1. Record the county.
2. Record the class.
3. Record the Program Year.
4. List all ACF enrollees registered on October 31 for the applicable Program Year.

 

1. Record the staff signature.
2. Record date staff signed form.

 

 

To be completed by Supervisor –   (Once form received need to hold until $ amount is received from WV Department of Transportation):

 

1. Record the transportation cost per pupil in the respective county for the applicable program year.
2. Record the number of enrollees for this class as of October 31.
3. Complete the formula: cost per pupil transported for program year multiplied by number of enrollees on October 31 equals the total in-kind value.

 

1. Record immediate Supervisor signature.
2. Record date immediate Supervisor signed form.
3. Record the total value.

 

 

This form will be completed annually prior to end of Program Year.
