INSTRUCTIONS FOR GIFTS, DONATIONS, AND/OR DONATED PROFESSIONAL SERVICES

GIFTS/DONATIONS:
- Record name of donor.
- Record home phone number of donor.
- Record work phone number of donor.
- Record address of donor.
- Identify gift or donation (example: Books for classroom).
- Record purpose of gift or donation (example: educational need).
- Record value of gift or donation (example: $500 – based on information from donor).
- Donor of gifts/donations will sign in ink.
- Date of signature.
OFFICE USE ONLY: This section is to be completed by staff only (#17, 18, 19, 20, 21, 22, & 23)
- Check mark HS or EHS for applicable program.
- Record the name of the site receiving donation.
- Record the location code.
- Check mark applicable donor source.
- Check which one that applies (In-kind or Resources).
- Employee signature accepting donation.
- Date of signature.
OFFICE USE ONLY: The section below (#24, 25, & 26) is to be completed by auditing staff only.
- Check mark the appropriate In-Kind category.
- Check mark the appropriate source of donation for Non In-Kind Resources.
- Record total value of Gift and/or Donation.
- Supervisor signature for approval.
- Date of signature
PROFESSIONAL SERVICES:
- Record date service provided.
- Record county in which service provided.
- Record name of professional providing the service.
- Record professional’s position and/or title.
- Brief description of service provided.
- Brief description of how hours/value determined.
- Record number of hours donated multiplied by the value amount = total value.
- Donor of gifts/donations will sign in ink.
- Date of signature.

DEFINITION OF IN-KIND CATEGORIES:
5408 | Space - Local Government | Space donated by County, City, and Boards of Education |
5409 | Space – Private | Space donated by private companies or individuals |
5410 | Local Government – Value of Contracted Services | Includes County, City, and Boards of Education. Contracts with these local governments such as an agreement to pay an agreed upon amount per child for services received. |
5411 | Local Government Donated Goods/Services Value | Includes County, City, and Boards of Education. The items donated could be teachers and principals time, any tangibles donated (food, books, items staying in classroom), transportation of children by bus to and from school by BOE, collaborations, and maintenance on buses/vehicles & diesel fuel. |
5412 | Value of Services from Businesses | Donation of services from a business for professional services and discounts |
5413 | Private – Value of Other Donated Items

The items donated are from an individual and can be any tangible items donated or discounted (such as food, books, items staying in classroom), collaborations with private child care centers, parent mileage, and tangible discounts. |
DEFINITION OF NON IN-KIND DONATED RESOURCES
(Definition: Services or Goods provided to the program or program participants that cannot be counted as in-kind contributions. These items do not provide a benefit in the classroom but benefit our HS/EHS children and/or parents outside the classroom.)
