Instructions for Administrative In-kind

Staff will use this form to document any Administrative in-kind. Administrative In-kind activities include participation/time spent on the Personnel{Hiring}/Community/Complaint Committee; Finance/Budget Committee; and Board of Directors.
- Record the month/year that form is being completed. Example: November 2011
- Record the type of activity. Example: Hiring Committee
- Record the date of the administrative activity. Example: 8/1/18
- Participant will print their name.
- Participant will sign their name.
- Record the number of minutes necessary to complete the activity.
- Record the allowable rate.
- Office Use Only – Designated staff will calculate total in-kind value.
- Staff records their signature in blue ink.
- Record date form is signed.
